About Consequence

Consequence UK Ltd, was established in 2003.

The company provides:

  • independent health and social care investigations;
  • statutory mental health investigations (HSG(94)27);
  • root cause analysis (RCA) and investigation training;
  • risk management training; and
  • facilitated risk assessments for individual teams and services.

Consequence has established itself as a company that

The managing director, Maria Dineen, worked closely with the National Patient Safety Agency in the design and testing of its approach to the investigation of serious adverse incidents in the National Health Service (NHS).
