Consequence

risk management disciplines, including root cause analysis
(RCA). It has been retained as the primary development
partner for the NPSA, producing the content for its prototype
RCA tool kit and delivering the RCA learning sets utilised
at NPSA pilot sites.

extensive expertise in the development of risk management
strategies and the role of risk management in clinical
governance, Consequence is adept at supporting the tactical
application of risk management strategies across every
level of the health care organisation.

The Director
With
18 years NHS experience, spanning both clinical and managerial
posts, Maria is a leading UK authority in risk management
within the healthcare sector. She has published widely
on clinical risk development and incident management and
is a regular speaker at national forums on these issues.
Maria organises and facilitates internal and external
workshops in Risk Assessment, Incident Management and
RCA.
As
co-author of the NPSA's initial guidance document 'Doing
Less Harm', Maria was also responsible for the creation
of the NPSA's initial risk assessment tool for grading
adverse incidents. An experienced trainer and coach, Maria
developed and facilitated the RCA learning sets provided
to NPSA pilot Trusts.
Maria
began her career in clinical risk in 1994 as the Perinatal
Outcomes Analyst at John Radcliffe Hospital, Oxford. She
then went on to become one of the first assessors for
the Clinical Negligence Scheme for Trusts.
Her
varied roles include Clinical Risk Manager at Worcester
Royal Infirmary NHS Trust, Research Fellow at the Health
Services Centre, Birmingham University, and Associate
Director of Clinical Governance at Oxfordshire Mental
Healthcare NHS Trust. In 2000 she founded Consequence.
Maria
is a member of the Institute of Risk Management and the
Health and Social Care Quality Network.
Publications
Maria has published widely on clinical risk development
and incident management in the UK since 1996. Her most
recent contribution to the field has been as co-author
of the Department of Health document - 'Doing Less Harm'.
Associates
