We deliver ‘on site’ and open learning workshops.
In house workshops
The four types of investigation workshops delivered on an ‘on client site’ basis are:
- a one-day awareness raising workshop;
- a two-day skills development workshop;
- a two plus one skills development workshop; and
- a Master Class.
All workshops can accommodate between 25 and 40 persons. Groups of 25 persons are led by a single facilitator and groups of between 26 – 40 persons are led by two facilitators.
The one-day workshop is designed to educate attendees in what constitutes an effective investigation. At the end of the workshop attendees will know the core steps in the investigation process and will have had the opportunity to work practically with some of the investigative tools and techniques.
The two day workshop is aimed at staff who are expected to lead moderate to serious incident investigations on behalf of their service or organisation. This includes staff who undertake safeguarding investigations.
The workshop programme explores the investigation process in detail and includes a high level of practical work throughout.
The areas addressed in the workshop are:
- commissioning, preparation, planning, communicating;
- being open with patients, families and carers;
- documentation review and information gathering – getting it right;
- analysing information gathered during the investigation process;
- determining root causes to any critical problems/failures;
- drawing conclusions and making effective recommendations; and
- writing the investigation report.
The two day workshop can be delivered over two consecutive days or on a split day basis. The latter option gives attendees the opportunity to implement in practice some of the principles covered on the first workshop day and to feed back their experiences on day two.
The two plus one workshop combines the best of the consecutive two day workshop plus a third reflective practice day some six to nine months after completion of day two. Attendees are expected to ‘buddy up’ and are tasked with undertaking at least one concise or comprehensive investigation during this time. On the reflective day participants present an overview of the investigation they conducted, reporting back on what tools and techniques they used and why. The reflective day also enables refresher topics to be identified in advance by the attendees and included in the day. NHS trusts who have used this workshop for their staff have indicated that they have found it beneficial in improving the conduct of internal investigations and the overall standard of investigation reports.
The Master Class is for individuals who have attended investigation and RCA training previously and have embedded their learning in the undertaking of Level Two investigations, preferably comprehensive investigations, although experience limited to concise investigations is fine.
Master Class participants are expected to:
- complete a pre-workshop questionnaire;
- prepare a 10-15 minute reflective practice presentation; and
- critique a comprehensive investigation report delivered.
