wishing to increase their base knowledge and skills in
the fields of Risk Management, Risk Assessment, Incident
Management and Root Cause Analysis. All case studies used
in the training programmes cover secondary acute, acute
mental health and primary care.
All participants
attending any of the RCA training programmes are provided
with the Six
Steps to Root Cause Analysis training manual.
This guide is an invaluable resource for those involved
in incident investigation and has been designed to provide
practical assistance when conducting an investigation,
and/or critical incident review using RCA methodologies.
It is particularly helpful to the novice investigator,
and those in the early stages of developing their root
cause analysis skills.

This day covers all of the essential elements of incident
investigation, the risk assessment of adverse events and
some basic easy to use, and apply, RCA tools at a superficial
level. It is ideal for staff that need to have an understanding
of the principles and concepts of RCA but who will not
be required to lead significant event investigations,
or facilitate critical incident reviews using RCA methodologies.
The day is exactly
what it purports to be - An Awareness Raising Day and
is not intended for staff who either have substantial
experience in significant event investigation, or who
have been appointed as Root Cause Analysis facilitators
and need to develop an appropriate skill base for this.

All three-day workshops are highly interactive. It
is therefore incumbent upon anyone joining this workshop
programme to be a fully participant member. All workshops
are run along the style of an action based learning set
as the absorption of skill and confidence in this field
comes as much from experience as it does learning the
theory.
During the breakout
sessions in the afternoons the group is split into two
and both groups are individually facilitated to enable
participants to benefit most from the skill and knowledge
of the facilitator and also to extract maximum benefit
from the experiences of self and others.
The workshop is
managed on a 'modular' basis over a period of not more
than 3 months (exact time span agreed with local organisations
where the course is run 'in-house') to enable participants
to 'practice' the skills and techniques taught and to
explore their experiences, successes and challenges at
subsequent days.
